Part-Time Licensed Clinical Social Worker (LCSW)

Compass Palliative Care is seeking a compassionate, licensed Clinical Social Worker to provide in-person support to patients in Assisted Living Facilities (ALFs).

Responsibilities

Provide social work assessments and support tailored to patients’ needs
Help patients and families navigate care resources and address psychosocial concerns
Coordinate with facility staff and the healthcare team
Maintain timely and accurate clinical documentation

Requirements

Active, unrestricted California LCSW license
Experience with older adults, medically complex patients, or palliative care preferred
Strong communication and organizational skills
Ability to work independently and travel to in-person visits

Position Details

Pay: $40–$45/hour
Schedule: 1 day per week | 8-10 hours per day
